TNF-α水平及基因多态性与种植体周围炎相关性分析

Correlation Analysis of TNF-α Level and Gene Polymorphism with Peri-implant Inflammation

摘要: 目的:分析肿瘤坏死因子-α(TNF-α)水平及基因多态性与种植体周围炎的相关性。方法:选取2021年1月至2022年6月在我院诊治的义齿种植修复患者136例作为研究对象,按照是否发生种植体周围炎分为炎症组(53例)和健康组(83例),另选取同期常规口腔检查的健康天然牙人群(68例)作为对照组,比较3组患者龈沟液TNF-α水平、牙周学指标以及TNF-α的基因型、等位基因的分布特点,并分析TNF-α水平与牙周学指标的相关性。结果:炎症组患者龈沟液TNF-ɑ水平、牙周学菌斑指数(PLI)、龈沟出血指数(SBI)、探诊深度(PD)均低于健康组、对照组患者(P<0.05)。经Pearson分析,龈沟液TNF-ɑ水平与牙周学指标PLI、SBI、PD均呈正相关(P<0.05)。三组患者TNF-α-308单核苷酸基因型频率和等位基因频率相比较,差异均有统计学意义(P<0.05),炎症组患者TNF-α-308单核苷酸等位A基因频率均低于健康组、对照组(P<0.05),等位G基因频率均高于健康组、对照组(P<0.05)。结论:种植体周围炎患者TNF-α水平呈高表达,且与病情严重程度有关,其发生和TNF-α-308单核苷酸基因多态性密切相关。

Abstract: Objective:To analyze the correlation of tumor necrosis factor-α (TNF-α) level and gene polymorphism with peri-implant inflammation. Methods:136 cases of denture implant restoration patients diagnosed and treated in our hospital from January 2021 to June 2022 were selected as the research object. According to whether peri-implant inflammation occurred, they were divided into the inflammation group(53 cases) and the healthy group (83 cases). In addition, the healthy natural teeth with routine oral examination at the same time were selected as the control group(68 cases). Three groups of gingival crevicular fluid TNF-α Level, periodontal indicators and the distribution characteristics of TNF-α genotype and allele were compared. The correlation between the level of TNF-α and periodontal index were analyzed. Results:The level of TNF-α in gingival crevicular fluid, periodontal plaque index (PLI), gingival sulcus bleeding index (SBI) and probing depth (PD) of inflammation group were lower than those of the healthy group and the control group (P<0.05). According to Pearson′s analysis, there was a positive correlation between TNF-α level in gingiral crevicularfluid, PLI, SBI and PD (P<0.05). The frequency of TNF-α 308 single nucleotide genotype of the three group had statisticall difference with that of alleles(P<0.05). The frequency of TNF-α 308 single nucleotide A gene of the inflammation group was lower than that of the healthy group and the control group (P<0.05), while the frequency of G gene was higher than that of the healthy group and the control group (P<0.05). Conclusion: TNF-α in patients with peri-implant inflammation is highly expressed and related to the severity of the disease. Its occurrence is closely related and TNF-α 308 single nucleotide gene polymorphism.
