MP-DNA载量联合T淋巴细胞亚群水平在小儿难治性肺炎支原体肺炎早期识别中的应用价值

The Application Value of MP-DNA Load Combined with T Lymphocyte Subsets in Early Diagnosis of Refractory Mycoplasma pneumoniae Pneumonia in Children

摘要: 目的:研究肺炎支原体(MP)-DNA载量、T淋巴细胞亚群水平在小儿难治性肺炎支原体肺炎(RMPP)早期识别中的应用价值。方法:纳入肺炎支原体肺炎(MPP)患儿128例,根据是否最终发展为RMPP,分为RMPP组和对照组,每组各64例。比较RMPP组和对照组患儿MP-DNA载量检测结果、T淋巴细胞亚群水平、白细胞计数以及淋巴细胞计数。使用ROC曲线分析MP-DNA载量、T淋巴细胞亚群水平、淋巴细胞计数以及MP-DNA载量联合T淋巴细胞亚群、淋巴细胞计数对RMPP早期诊断的价值。结果: RMPP组MP-DNA高载量率高于对照组(P<0.05)。RMPP组CD3+、CD4+、CD4+/CD8+、CD19+CD23+、淋巴细胞计数水平均低于对照组,CD56+水平高于对照组(P<0.05)。单独诊断中,CD4+对RMPP的诊断价值最高,AUC为0.793。MP-DNA高载量联合CD4+在RMPP早期识别中的应用价值更高,AUC为0.828。结论: 与普通MPP相比,RMPP患儿MP-DNA高载量发生率高,且T淋巴细胞亚群紊乱更为严重;其中MP-DNA高载量联合CD4+对RMPP早期识别应用价值更高。

Abstract: Objective: To observe the application value of Mycoplasma pneumoniae (MP)-DNA load and T lymphocyte subsets in the early diagnosis of refractory Mycoplasma pneumoniae pneumonia (RMPP) in children. Methods: 128 children with Mycoplasma pneumoniae pneumonia (MPP) were enrolled, and were divided into the RMPP group and the control group according to whether finally developed to RMPP, with 64 cases in each group. The levels of MP-DNA load, T lymphocyte subsets, white blood cell count, as well as lymphocyte count were compared between the RMPP group and the control group. ROC curve was used to analyze the value of MP-DNA load, T lymphocyte subsets, lymphocyte count, as well as MP-DNA load combined with T lymphocyte subsets, lymphocyte count in the early diagnosis of RMPP. Results: The incidence of MP-DNA high load in the RMPP group was obviously higher than that of the control group (P<0.05). The levels of CD3+, CD4+, CD4+/CD8+, CD19+CD23+ and lymphocyte count in the RMPP group were lower than those in the control group, and the level of CD56+ was higher than that of the control group(P<0.05). In the diagnosis of RMPP by single index, the diagnostic value of CD4+ was the highest (AUC=0.793). MP-DNA high load combined with CD4+ had higher value in the early diagnosis of RMPP (AUC=0.828). Conclusion: Compared with ordinary MPP, children with RMPP have higher incidence of MP-DNA high load, and the disorder of T-lymphocyte subsets is more serious, and MP-DNA high load combined with CD4+ has higher application value for early diagnosis of RMPP.
