百令胶囊联合羧甲司坦治疗AECOPD的疗效及对EOS、PCT、肺功能及应激反应指标的影响#br#

Efficacy of Bailing Capsule Combined with Carbocisteine in the Treatment of AECOPD and Its Effects on EOS, PCT, Lung Function and Stress Response#br#

摘要: 目的:探讨百令胶囊联合羧甲司坦治疗急性加重期慢性阻塞性肺疾病(AECOPD)的疗效及对嗜酸性粒细胞(EOS)、降钙素原(PCT)、肺功能及应激反应的影响。方法:以我院2021年3月至2023年3月收治的痰湿阻肺兼肺肾气虚证AECOPD患者112例为研究对象,按随机数字表法分为对照组及观察组,每组各56例。对照组在常规治疗基础上,予以口服羧甲司坦片;观察组在对照组治疗基础上,联合百令胶囊治疗。两组患者均治疗1周。比较两组患者的中医证候积分、肺功能、炎症因子、应激反应、免疫功能相关指标水平及不良反应发生率。结果:与治疗前相比,治疗后两组患者咳嗽气喘、痰多白粘、胃脘痞满及舌淡苔白等证候积分均降低,且观察组以上证候积分均低于对照组(P<0.05);两组患者第1秒用力呼气量(FEV1)、FEV1占预计值百分比(FEV1pred%)、用力呼气量(FVC)均有升高,且观察组以上指标均高于对照组(P<0.05);两组患者EOS、PCT水平均下降,且观察组EOS、PCT水平均低于对照组(P<0.05);两组患者丙二醛(MDA)水平均降低,超氧化物歧化酶(SOD)水平均上升,且观察组MAD水平低于对照组,SOD水平高于对照组(P<0.05);两组患者CD4+、CD4+/CD8+水平均升高,CD8+水平则降低,且观察组CD4+、CD4+/CD8+水平高于对照组,CD8+水平低于对照组(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:应用百令胶囊联合羧甲司坦治疗AECOPD疗效确切,患者肺功能增强,炎症及应激反应有所减轻,免疫功能提高,且不良反应较少。

Abstract: Objective: To investigate the efficacy of Bailing capsule combined with carbocisteine in the treatment of acute exacerbation chronic obstructive pulmonary disease (AECOPD) and its effects on eosinophils (EOS), procalcitonin (PCT), lung function and stress response. Methods: A total of 112 AECOPD patients with the syndrome of phlegm-dampness accumulated in the lung and the syndrome of lung-kidney Qi deficiency treated in our hospital from March 2021 to March 2023 were selected as the research objects, and were divided into the control group and the observation group according to random number table method, with 56 cases in each group. The control group was given carbocisteine tablets on the basis of conventional symptomatic treatment, and the observation group was treated with Bailing capsule on the basis of the treatment of the control group. Both groups were treated for 1 week. The TCM syndrome scores, lung function, inflammatory factors, stress response, immune function levels and the incidence of adverse reactions of the two groups of patients were compared. Results: The syndrome scores of cough and asthma, white sticky phlegm, distension in the stomach and white tongue coating of patients in both groups decreased after treatment, and the scores of the observation group were lower than those of the control group after treatment(P<0.05) . FEV1, FEV1 pred% and FVC of patients in both groups increased after the treatment, and the above indexes of the observation group was higher than those of the control group (P<0.05). The level of EOS and PCT of the two groups of patients decreased after the treatment, and the EOS and PCT levels of patients in the observation group was lower than those of the control group (P<0.05). The MDA level decreased and SOD level increased in both groups after the treatment, and the MAD level of patients in the observation group was lower than that of the control group, while the SOD level of patients in the observation group was higher than that of the control group (P<0.05). The levels of CD4+ and CD4+/CD8+ increased in both groups, while the level of CD8+ decreased in both groups, and the CD4+ and CD4+/CD8+ levels of patients in the observation group were higher than those of the control group, CD8+ level of patients in the observation group was lower than that of the control group (P<0.05). There was no statistical difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Bailing capsule combined with carbocisteine is effective in the treatment of AECOPD, which can improve the lung function, reduce inflammation and stress response, improve immune function of the patients, and with low incidence of adverse reactions .
