摘要/Abstract

摘要： 目的：了解上海市崇明区执行延伸处方政策的现状与存在的问题，为推进崇明区延伸处方的进一步开展提供参考意见。方法：在质性访谈基础上设计延伸处方服务主体以及服务对象的调查问卷，采用抽样调查的方式分别对家庭医生、药师、签约患者进行问卷调查，将调查结果进行归纳、总结。结果：共收集到237位家庭医生、96位药师、302位签约患者的有效问卷。调查结果显示，崇明区延伸处方政策的执行患者总体满意度高，但在执行过程中，仍存在需要改进的方面：社区卫生服务中心团队的专业能力有待提高；药师应真正融入到家庭医生的治疗团队中，为患者的安全用药保驾护航；应增设延伸处方的前置审方软件接入，通过信息化支撑助力合理用药；社区卫生服务中心应做实处方点评工作，持续提高医疗质量。结论：通过调查结果呈现的问题提出相关建议：提高医务人员的专业能力；增加对医务人员的激励机制；做实处方点评工作，提高医疗质量；扩大宣传，提高家庭医生服务签约率；简化药品相关代码信息，扩大延伸处方药品目录，稳定物流保障供货，多措并举推进崇明区延伸处方政策的执行，并以此推动上海市分级医疗服务体系的构建。

Abstract: Objective：To investigate the current status and existing problems of the implementation of extended prescription policy in Shanghai Chongming district，and to provide reference for promoting further development of extended prescription in Chongming district.Methods：Based on qualitative interviews， the questionnaires for the service subject and service object of extended prescription were designed.Questionnaire survey was conducted among family doctors， pharmacists and contracted patients by means of sampling survey， and the survey results were summarized and analyzed.Results：Valid questionnaires were collected from 237 family doctors， 96 pharmacists and 302 contracted patients. The survey results showed that the overall satisfaction rate of patients to the implementation of the extended prescription policy in Chongming district was high， but there were still some aspects to be improved in the implementation process.The professional competence of the medical team of community health service center needs to be improved.Pharmacists should integrate into the treatment team of family doctors to ensure medication safety for the patients.It is necessary to add the pre-prescription review system for extended prescriptions to facilitate rational drug use through information support.The community health service center should carry out prescription evaluation for extended prescriptions to continuously improve the quality of medical treatment.Conclusion：According to the problems presented by the survey results， relevant suggestions were put forward， including improving the professional competence of medical staffs， improving the incentive mechanism for medical staffs， carrying out prescription evaluation to improve the quality of medical care， expanding publicity to increase the rate of signing up for family doctor services， simplifing drug code information， expanding the extended prescription drug catalog， stabling logistics and ensuring supply， so that to promote the implementation of extended prescription policy in Chongming district， and to promote the construction of hierarchical medical service system in Shanghai.
