体细胞型纵隔恶性畸胎瘤病例分析

摘要/Abstract

摘要： 畸胎瘤是由不同胚胎层组成的胚胎肿瘤，好发于性腺，纵隔是最常见的性腺外部位。体细胞型纵隔恶性畸胎瘤是一种罕见的纵隔畸胎瘤，发病隐匿，临床表现不典型，早期可无症状，随着肿瘤增大可出现胸闷、胸痛、声音嘶哑、呼吸困难等症状，影像学表现缺乏特异性，临床上容易误诊和漏诊，暂无规范的专家共识及治疗指南，目前以综合治疗为主，患者预后较差，生存期短。因此，个体化的治疗方案可能提高治疗效果，改善生存预后，延长生存期。本研究通过报道1例绝经后女性体细胞型纵隔恶性畸胎瘤患者的诊治经过，并通过文献复习总结探讨此类肿瘤的诊断及治疗现状，以期为临床治疗体细胞型纵隔恶性畸胎瘤提供参考。

Abstract: Teratoma is an embryonic tumor composed of different embryonic layers. It usually occurs in the gonad. The mediastinum is the most common extragonadal site. Malignant somatic teratoma of the mediastinum is a rare type of mediastinal teratoma with occult onset and atypical clinical manifestations. It may be asymptomatic in the early stage， and may present chest tightness， chest pain， hoarseness， and dyspnea as the tumor grows.Its imaging manifestations lack specificity， and is easy to be misdiagnose and missed diagnosis.Currently， there is no normative expert consensus and treatment guidelines， combined treatment is the main treatment method， and the prognosis of patients is poor and the survival time is short. Therefore， individualized treatment may improve the therapeutic effect， improve the survival prognosis and prolong the survival time.This study reports the diagnosis and treatment of a postmenopausal female patient with somatic mediastinal malignant teratoma and summarized the diagnosis and treatment status of such tumors through literature review，so as to provide reference for clinical treatment of malignant somatic teratoma of mediastinum.
