虚拟现实技术联合康复功能训练对偏瘫患者DTI功能成像和上肢运动功能的影响

Effect of Virtual Reality Technology Combined with Rehabilitation Training on DTI Functional Imaging and Upper Limb Motor Function in Patients with Hemiplegia

摘要: 目的:研究虚拟现实技术(VR)联合康复功能训练对偏瘫患者弥散张量成像(DTI)功能成像和上肢运动功能的影响。方法:选择84例2019年2月至2021年10月收治的偏瘫患者作为研究对象。采用随机数字法将其分成联合组及常规组,每组各42例。常规组开展常规康复功能训练,联合组则在常规组基础上增用VR干预。比较两组患者肩肘关节活动情况、DTI功能成像、上肢运动功能、日常生活活动能力、心理状态。结果:治疗后两组患者各项肩肘关节活动度均高于治疗前,且联合组相较常规组活动度更高(P<0.05)。两组患者治疗后放射冠层面的各向异性分数(FA)、FA比值(rFA)均明显升高;且放射冠层面FA不对称性(FAasy)值均降低(P<0.05),且联合组改善更明显(P<0.05)。治疗后两组患者Fugl-Meyer运动功能评分(FMA)上肢以及巴氏指数(BI)均高于治疗前,且联合组均高于常规组(P<0.05)。治疗后联合组及常规组SAS、SDS评分均低于治疗前,且联合组低于常规组(P<0.05)。结论:VR联合康复功能训练对偏瘫患者DTI功能成像具有一定的改善作用,且有效改善上肢运动功能,提升患者肩肘关节活动度,减轻焦虑、抑郁情绪。

Abstract: Objective: To study the effects of virtual reality technology (VR) combined with rehabilitation training on diffusion tensor imaging (DTI) functional imaging and upper limb motor function in patients with hemiplegia. Methods: A total of 84 patients with hemiplegia admitted to the hospital from February 2019 to October 2021 were included in the study, and were divided into the combined group and the conventional group according to random number table method, with 42 cases in each group. The conventional group received routine rehabilitation training, while the combined group received VR intervention on the basis of the conventional group. The range of motion of shoulder and elbow joint, DTI functional imaging, upper limb motor function, activities of daily living, and psychological status. were compared between the two groups. Results: After treatment, the range of motion of shoulder and elbow joint of patients in the two groups was higher than that before treatment, and the combined group was higher than the conventional group (P<0.05). After treatment, the fractional anisotropy (FA) and the ratio of FA (rFA) at the coronal level were significantly increased in both groups. The values of FA asymmetry (FAasy) at the corona radiata level were significantly decreased (P<0.05). After treatment, the Fugl-Meyer motor function score (FMA) of upper limb and BI of the two groups were higher than those before treatment, and the combined group was higher than that of the conventional group (P<0.05). After treatment, the scores of SAS and SDS of patients in the combined group and the conventional group were lower than those before treatment, and the combined group was lower than the conventional group (P<0.05). Conclusion: VR combined with rehabilitation functional training has certain improvement effect on DTI functional imaging in patients with hemiplegia, and can effectively improve upper limb motor function, improve shoulder and elbow joint range of motion, and reduce anxiety and depression.
