大柴胡汤联合熊去氧胆酸对LCBDE术后胆汁代谢的影响

Effects of Dachaihu Decoction Combined with Ursodeoxycholic Acid on Bile Metabolism after LCBDE

摘要: 目的:评估大柴胡汤联合熊去氧胆酸对胆总管结石患者胆总管切开探查取石术(LCBDE)术后胆汁代谢的影响。方法:选取2022年1月至2022年12月于我院住院的胆总管结石患者50例,随机分为观察组和对照组,每组各25例。对照组采用LCBDE、熊去氧胆酸治疗;观察组在对照组治疗方案的基础上增加大柴胡汤治疗。结果:术后4、8 d时,观察组患者血清谷丙转氨酶、谷草转氨酶水平均低于对照组(P<0.05),胆汁中总胆红素、间接胆红素水平均低于对照组(P<0.05)。治疗后各时间点,两组患者胆汁中总胆固醇和总胆汁酸水平比较,差异均无统计学意义(P>0.05)。术后8 d,两组患者胆汁中总胆固醇水平均低于术前4 d(P<0.05),总胆汁酸水平均高于术前4 d(P<0.05)。结论:大柴胡汤能加速LCBDE术后患者胆红素的代谢;熊去氧胆酸能增加LCBDE术后患者胆汁酸的分泌,降低胆固醇水平。大柴胡汤和熊去氧胆酸联合应用,能改善胆总管结石患者LCBDE术后胆汁代谢,从而加速患者术后肝功能恢复。

Abstract: Objective: To evaluate the effects of Dachaihu decoction combined with ursodeoxycholic acid on bile metabolism after LCBDE in patients with choledocholithiasis. Methods: A total of 50 patients with choledocholithiasis who were hospitalized in our hospital from January 2022 to December 2022 were randomly divided into the observation group (n=25) and the control group (n=25). The control group was treated with LCBDE and ursodeoxycholic acid, and the observation group was treated with Dachaihu decoction on the basis of the control group. Results: After treatment, the levels of serum glutamic pyruvic transaminase and serum glutamic oxaloacetic transaminase of patients in the observation group were lower than those of the control group (P<0.05), and the levels of total bilirubin and indirect bilirubin in bile of patients in the observation group were lower than those in the control group (P<0.05). On the time of 8 d after operation, the total cholesterol level in bile of both groups was lower than those of 4 d after operation (P<0.05), and the total bile acid level of both groups was higher than those of 4 d after operation (P<0.05).Conclusion: Dachaihu decoction can accelerate the metabolism of bilirubin in patients after LCBDE. Ursodeoxycholic acid can increase bile acid secretion and reduce cholesterol level in patients after LCBDE. The combined application of Dachaihu decoction and ursodeoxycholic acid can improve bile metabolism in patients with choledocholithiasis after LCBDE, so that speeding up the recovery of liver function.
