Abstract:

Objective： To explore the application effect of project-achieving quality control circle（QCC）in the construction of a new discharge medication mode based on patient needs.Methods： The QCC team was established and the theme of the QCC activity was "building a new discharge medication mode based on patient needs". Strictly implement the PDCA cycle and the 10 steps of project-achieving QCC to carry out activities. The tangible and intangible results after the implementation of QCC activities were evaluated.Results： In terms of tangible achievements， after the implementation of QCC activities，the average waiting time of patients in peak period was reduced from 45 minutes to 15.4 minutes， The patients' satisfaction with discharged medication service increased from 90.09% to 98%. The omission rate of taking medicine at discharge decreased from 4.75% to 0.29%. The average time for filling prescriptions in peak period reduced from 5 minutes to 2.5 minutes and the rate of dispensing errors reduced from 0.417% to 0%. In terms of intangible achievements， after the implementation of QCC activities，the QCC members' scores of the sense of responsibility， sense of positivity， self-confidence，expanding thinking， skills of QCC， skills of communication and coordination， team spirit and problem-solving ability improved compared with those before the implementation.Conclusion： In this study， the new discharge medication mode based on patient needs contructed by project-achieving QCC can effectively improve the quality of pharmaceutical care and the comprehensive quality and ability of pharmacists.
