益气醒脑饮联合VR技术指导的功能锻炼对老年缺血性脑卒中后下肢功能障碍患者的影响

Effect of Yiqi Xingnao Yin Combined with Functional Exercise Guided by VR Technology on Elderly Patients with Lower Limb Dysfunction After Ischemic Stroke

摘要:

目的:探讨益气醒脑饮联合VR技术指导的功能锻炼对老年缺血性脑卒中后下肢功能障碍患者的影响。方法:选取20177月至20227月我院诊治的90例老年缺血性脑卒中后下肢功能障碍患者,随机分为益气醒脑饮组(A组)、益气醒脑饮联合VR技术指导的功能锻炼联合治疗组(B组)和VR技术指导的功能锻炼组(C组)3组。A组患者口服益气醒脑饮(20 mL·d-1tid),B组给予益气醒脑饮(20 mL·d-1tid)联合VR技术指导的功能锻炼(每天1次),C组仅作VR技术指导的功能锻炼治疗,每天1次。治疗3个月后评估3组临床疗效,比较治疗前后各组患者傅格-梅尔氏运动量表(FMMS)评分、下肢肌力、神经功能缺损程度、运动能力、日常生活能力以及血脂变化情况。结果:3组治疗有效率分别为78.57%89.66%67.80%,均能改善缺血性脑卒中患者的下肢功能障碍,组间比较差异有统计学意义(P<0.05)。与治疗前比较,治疗后各组患者FMMS评分升高、下肢肌力评分下降(P<0.05),且BFMMS评分提高、下肢肌力评分下降幅度更大(P<0.05)。治疗后各组患者的NIHSS评分和CSS评分低于治疗前(P<0.05),B组患者治疗后的NIHSS评分和CSS评分下降程度更多(P<0.05),3组患者治疗后BI指数评分未见显著差异。与治疗前比,治疗后AB组患者CHOTGLDL水平均降低(P<0.05),且与C组患者比较差异有统计学意义(P<0.05),各组患者的HDL水平差异无统计学意义。结论:益气醒脑饮联合VR技术指导的功能锻炼可以改善老年缺血性脑卒中后下肢功能障碍患者的运动能力,降低体内CHOTGLDL的水平,提高患者日常生活能力。

Abstract:

Objective: To explore the effect of yiqi xingnao yinYQXN combined with functional exercise guided by VR technology on elderly patients with lower limb dysfunction after ischemic stroke.Methods: A total of 90 elderly patients with lower limb dysfunction after ischemic stroke treated in our hospital from July 2017 to July 2022 were selected. The patients were randomized divided into 3 groups namely yiqi xingnao yin group Group A), yiqi xingnao yin combined with VR-guided functional exercise group Group B and VR-guided functional exercise group Group C. The patients in group A were treated with yiqi xingnao yin 20 mL·d-1 tid while group B were given the combined treatment of yiqi xingnao yin 20 mL·d-1 tid combined with VR guided functional exercises once a day), and the patients in group C received VR-guided functional exercise therapy once a day. After 3 months of treatment the clinical efficacy of the three groups was evaluated and the FMMS score lower limb muscle strength degree of neurological deficit motor ability daily living ability and changes in blood lipids of patients in each group were compared before and after treatment.Results: The treatment efficacy of the three groups was 78.57% 89.66% and 67.80% respectively all of which demonstrated the capacity to enhance lower limb functionality in ischemic stroke patients and there was no statistial difference between the groups. FMMS scores were significantly higher and lower limb muscle strength scores were lower in all groups after treatment compared to pre-treatment P<0.05), and FMMS scores were significantly higher and lower limb muscle strength score was lower in group B after treatment P<0.05. NIHSS scores and CSS scores were lower in all groups after treatment than before treatment P<0.05. The NIHSS scores and CSS scores of group B patients decreased more significantly after treatment P<0.05. There was no significant difference in the scores of the BI index among the three groups of patients after the treatment. Compared with pre-treatment the levels of CHO TG and LDL of patients in group A and group B reduced after treatment P<0.05 and there were statistical difference with those in group C P<0.05), and there was no significant difference in the levels of HDL between the groups.Conclusion: Yiqi xingnao yin combined with VR guided functional exercise can improve the motor ability of elderly patients with lower limb dysfunction after ischemic stroke reduce levels of CHO TG and LDL in the body and improve the patient's ability of daily life.
