中医药调控M1/M2巨噬细胞极化治疗类风湿关节炎研究进展

摘要:

类风湿关节炎(rheumatoid arthritisRA)作为慢性系统性自身免疫病的典型代表,其核心病理特征表现为滑膜组织反复持续的炎症反应。巨噬细胞是免疫系统的重要组成部分,具有调节免疫反应、参与机体的防御和修复等多种功能,凭借其表型可塑性在免疫稳态调控中发挥枢纽作用,并且其在不同的微环境下极化为促炎的M1型巨噬细胞和抑炎的M2型巨噬细胞。M1M2处于相互制约的平衡状态,两者的动态平衡构成滑膜微环境稳定的基础。这种对立制约的关系与中医学的核心理论“阴阳平衡”有异曲同工之妙。近年来,随着对巨噬细胞极化研究的不断深入,发现中医药以其多成分、多靶点、多途径的综合作用调节巨噬细胞不同表型的极化,在RA治疗中发挥关键作用。基于此,本研究概述了巨噬细胞极化在RA中的作用,并总结了中药提取物、中药活性成分及中药复方通过调控M1/M2巨噬细胞极化治疗RA的研究进展,以期为寻找RA潜在的新型靶向疗法提供科学依据和理论支撑,并为其的临床研究和药物研制提供新的思路。

Abstract:

Rheumatoid arthritis RA), as a typical representative of chronic systemic autoimmune diseases is characterized by repeated and persistent inflammatory responses in the synovial tissue. Macrophages an important component of the immune system have multiple functions such as regulating immune responses participating in the defense and repair of the body and play a pivotal role in immune homeostasis regulation due to their phenotypic plasticity. They polarize into pro-inflammatory M1 macrophages and anti-inflammatory M2 macrophages in different microenvironments where M1 and M2 maintain a mutually restrictive balance. The dynamic balance between the two constitutes the basis for the stability of the synovial microenvironment which is a phenomenon that parallels the core theory of "yin-yang balance" in traditional Chinese medicineTCM.In recent years continuous in-depth research on macrophage polarization has revealed that TCM regulates the polarization of different macrophage phenotypes through its multi-component multi-target and multi-pathway comprehensive effects thereby playing a key role in RA treatment. Based on these findings this article systematically summarizes the role of macrophage polarization in RA pathogenesis and reviews recent advancements in TCM-based interventionsincluding traditional Chinese medicine extracts active components of TCM and TCM formulas all of which achieve therapeutic effects by modulating M1/M2 macrophage polarization. The primary objective of this work is to provide a scientific foundation for identifying potential novel targeted therapies against RA while simultaneously offering innovative perspectives to guide clinical research and pharmacological development in this field.
