米索前列醇对妊娠期高血压产妇血清视黄醇结合蛋白及凝血功能的影响

Effects of Misoprostol on Serum Retinol Binding Protein and Coagulation Function in Puerperas with Pregnancy Induced Hypertension

摘要: 目的:探究米索前列醇对妊娠期高血压产妇血清视黄醇结合蛋白及凝血功能的影响。方法:选取2018年3月至2019年5月我院妇产科收治的妊娠期高血压产妇104例,随机分为对照组(n=52)和观察组(n=52)。对照组接受常规缩宫素治疗,观察组在此基础上加用米索前列醇治疗,观察两组血清视黄醇结合蛋白(RBP)和凝血功能水平变化,同时比较两组于分娩后即刻和治疗后24h收缩压、舒张压及产后出血量(24 h)。结果:观察组RBP水平较对照组更低(P<0.01);治疗后,观察组凝血指标凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(APTT)时间均低于对照组,且血浆纤维蛋白原 (FIB)升高更明显(P<0.01);治疗后,观察组收缩压、舒张压和产后出血量均小于对照组(P<0.01)。结论:应用米索前列醇治疗妊娠期高血压产妇,可调节凝血系统和RBP水平,同时有效降低产后出血风险。

Abstract: Objective:To explore the effects of misoprostol on serum retinol binding protein and coagulation function in puerperas with pregnancy induced hypertension. Methods: A total of 104 puerperas with pregnancy induced hypertension who were treated in obstetrics and gynecology department in our hospital from March 2018 to May 2019 were randomly divided into control group (n=52) and observation group (n=52). The control group received conventional oxytocin treatment, and the observation group was treated with misoprostol on that baisis. The changes in serum retinol binding protein (RBP) and coagulation function were observed in the two groups. The systolic blood, diastolic pressure, and postpartum hemorrhage(24 h) of the two groups were compared immediately after delivery and 24 hours after treatment. Results:The RBP level in the observation group was lower than that in the control group (P<0.01). After treatment, the systolic blood pressure, diastolic blood pressure, and postpartum hemorrhage in the observation group were all lower than those in the control group (P<0.01). After treatment, the coagulation indexes PT, TT, and APTT in the observation group were significantly lower than those in the control group, and the FIB increased more significantly (P<0.01). Conclusion:The application of misoprostol in the treatment of puerperas with pregnancy induced hypertension can regulate the blood coagulation system and RBP levels, and effectively reduce the risk of postpartum hemorrhage.
