药物临床试验研究中心筛选量表的编制与验证研究

摘要:

目的:设计可量化的药物临床试验研究中心筛选评估量表,并进行实践验证,以提高药物临床试验研究中心的筛选质量。方法:文献研究结合行业专家意见,构建药物临床试验研究中心筛选的指标体系,设计其筛选评估量表。以SPSS软件统计分析量表信度水平,专家评价法评估量表的内容效度水平。量表应用于创新药TBN治疗肌萎缩侧索硬化症(ALS)的期临床试验中以验证其可行性。结果:初步构建的《研究中心筛选评估量表》共有4个维度、26个指标。量表预评估阶段共收集41家研究中心数据,Cronbach α系数为0.939,信度水平较高;内容效度水平0.84,效度水平较高;删减效度水平较差的5个指标后,正式量表最终保留4个维度21个指标。在TBN治疗ALS期临床试验中开展正式量表的具体实施,有74家研究中心可开展ALS项目;Cronbach α0.927,信度水平较高。按照量表评分将研究中心分为高分组、中分组、低分组,对比并分析4家研究中心试验进度与质量情况,结果表明高分组研究中心试验进度更快,试验质量问题如知情同意书签署不规范、安全性事件漏报迟报、试验药品超温或分发接收有误的发生率更低。结论:通过实例检验,可量化的《研究中心筛选评估量表》对临床试验研究中心的筛选质量管理具有参考价值。

Abstract:

Objective: To design and validate an assessment scale for the selection of drug clinical trial sites so as to ensure the quality of site selection.Methods: The index system for site selection of drug clinical trials was constructed based on expert opinions and literature review and an assessment scale was designed using this index system. The reliability of the scale was analyzed by SPSS software and the content validity of the scale was assessed by experts. The scale was then applied in a phase Ⅱ clinical trial of TBN for ALS to validate its feasibility.Results: The preliminary scale consisted of 4 dimensions and 26 indicators. A total of 41 sites were included in the pre-assessment stage of the scale and the Cronbach's alpha coefficient was 0.939 indicating high reliability. The content validity index was 0.84 indicating good validity. After removing 5 indicators with poor validity the final scale consisted of 4 dimensions and 21 indicators. In the phase Ⅱ clinical trial of TBN treatment for ALS 74 sites were available for ALS research. The scale was applied and the Cronbach's alpha coefficient was 0.927. According to the scale scores the sites included were divided into high score group middle score group and low score group and the clinical trial progress and quality of four sites were compared and analyzed. The results showed that the sites in the high score group had faster trail progress and lower incidence of trail quality problems such as non-standard informed consent signing missed and delayed safety event reporting overtemperature of the investigational drug and incorrect distribution and reception of the investigational drug.Conclusion: The "site selection assessment scale" developed in this study which quantifies the impact index of clinical trial site selection management has significant reference value for the quality management in clinical trial site selection.
