编织钢丝绳重建外侧壁结合PFNA治疗A3型股骨转子间骨折的临床疗效分析

Clinical Efficacy of Braided Wire Rope Reconstruction of the Lateral Wall Combined with PFNA in the Treatment of A3 Type Intertrochanteric Femur Fractures

摘要:

目的:探讨编织钢丝绳重建外侧壁结合防旋型股骨近端髓内钉(PFNA)治疗A3型股骨转子间骨折的疗效。方法:选取我院20162022年通过PFNA治疗的A3型股骨转子间骨折患者52例,随机分成两组,其中A组患者采用编织钢丝绳重建外侧壁结合PFNA治疗,共26例(男性14例,女性12例);B组患者单纯采用PFNA治疗,共26例(男性12例,女性14例);分别完成2年随访。比较两组患者的手术时长、术中出血量、术后15 d数字评价量表疼痛评分(NRS)、内侧壁复位质量、尖顶距、头颈螺钉位置、住院天数、骨折愈合时间、内固定失败例数、术后完全负重时间以及随访终期髋关节功能评分(Harris评分)。结果:A组与B组患者在手术时长、术中出血量、术后15 d疼痛评分、内侧壁复位质量、尖顶距、头颈螺钉位置及住院天数方面比较,差异无统计学意义(P>0.05)。A组患者骨折愈合时间为(8.42±3.23)周,内固定失败2例,术后完全负重时间为(37.29±4.54 d,末次随访时髋关节功能优良率为84.62%B组患者骨折愈合时间为(13.58 ± 3.92)周,内固定失败8例,术后完全负重时间为(53.61±8.71 d,末次随访时髋关节功能优良率为57.69%A组与B组在骨折愈合时间、内固定失败例数、术后完全负重时间及髋关节功能优良率比较,差异均有统计学意义(P<0.05)。结论:编织钢丝绳重建外侧壁结合PFNA治疗A3型股骨转子间骨折较单纯使用PFNA临床疗效更好,内固定失效发生少。

Abstract:

Objective: To investigate the efficacy of braided wire rope reconstruction of the lateral wall combined with PFNA in the treatment of A3 type intertrochanteric femur fractures.Methods: A total of 52 patients with A3 type intertrochanteric femur fractures treated with PFNA in our hospital from 2016 to 2022 were randomly divided into two gropus. Group A 26 cases 14 males and 12 females underwent braided wire rope reconstruction of the lateral wall combined with PFNA. Group B 26 cases 12 males and 14 females was treated with PFNA alone. Two-year follow-up was completed for each group. The operation duration intraoperative bleeding numerical rating scale NRS pain score at 15 days after the surgery quality of medial wall reset tip-apex distance position of lag screw length of hospital stay fracture healing time cases of internal fixation failure time to full weight-bearing after the surgery and the final follow-up hip function score Harris score were compared between the two groups.Results: There were no statistically significant differences between group A and group B in operation duration intraoperative bleeding pain score at 15 days after the surgery quality of medial wall reset tip-apex distance position of lag screw and length of hospital stay P > 0.05. In group A the mean fracture healing time was 8.42 ± 3.23 weeks with 2 cases of internal fixation failure the time to full weight-bearing after the surgery was 37.29 ± 4.54 days and the excellent rate of hip function at the last follow-up was 84.62%. While in group B the mean fracture healing time was 13.58 ± 3.92 weeks with 8 cases of internal fixation failure the time to full weight-bearing after the surgery was 53.61 ± 8.71 days and the excellent rate of hip function at the last follow-up was 57.69%. There were statistically significant differences between group A and group B in fracture healing time the number of internal fixation failures time to full weight-bearing after the surgery and the excellent rate of hip function after the surgery P < 0.05.Conclusion: Braided wire rope reconstruction of the lateral wall combined with PFNA is more effective and less incidence of internal fixation failure than PFNA alone in the treatment of A3 type intertrochanteric femur fractures.
