摘要/Abstract

摘要： 目的：探讨老年慢性心力衰竭（CHF）患者实施延续性护理模式对其生活质量的影响。方法：将90例老年慢性心力衰竭患者随机分为观察组和对照组，各45例。对照组给予常规出院指导及回访，观察组出院后实施延续性护理，延续到患者出院后6个月。于出院时，出院后1、3、6个月分别评估两组患者生活质量、自我管理能力、卫生服务利用等指标。结果：观察组患者出院后1、3、6个月的生活质量和自我管理能力评分与对照组相比均有改善(P＜0.05)。观察组门诊次数、急诊次数、住院卫生服务次数低于对照组。结论：应用延续性护理模式能够对老年慢性心力衰竭患者进行有效的管理，能改善患者生活质量。

Abstract: Objective：To explore the effect of continuous nursing mode on the quality of life of elderly patients with chronic heart failure. Methods： 90 cases of elderly patients with chronic heart failure were divided into observation group and control group with 45 cases in each group. The control group was given routine discharge guidance and visits，the observation group implemented continuous nursing after discharge， until 6 months after hospital discharge.Two groups of patients′ quality of life， self management ability and health service utilization index were assessed respectively during the period of 1，3，6 months after hospital discharge. Results： After 1，3，6 months， the patients′ quality of life and self management ability score in observation group were better than those of the control group(P<0.05). The number of outpatient visits， the number of emergency visits， the number of hospital health services in the observation group were significantly lower than those in the control group. Conclusion： The application of continuity of care model can effectively manage the elderly patients with chronic heart failure，and can improve the patients′ quality of life.
