摘要: 【摘要】目的:探讨慢性阻塞性肺疾病(COPD)合并呼吸衰竭有创和无创呼吸机序贯治疗的临床效果。方法:COPD合并呼吸衰竭患者120例,随机分为A组(60例)和B组(60例)。B组进行有创机械通气至脱机,A组在有创机械通气后序贯进行无创机械通气。比较两组的疗效,观察两组患者的呼吸机相关性肺炎(VAP)发生率、总机械通气时间、有创通气时间、住院时间和撤机时间。结果:A组的总有效率、VAP发生率、总机械通气时间、有创通气时间、住院时间和撤机时间均明显优于B组(P<0.05)。结论:COPD合并呼吸衰竭有创和无创呼吸机序贯治疗疗效优于单纯有创呼吸机组。

Abstract: 【ABSTRACT】Objective: To investigate the clinical effect of sequential therapy of chronic obstructive pulmonary disease(COPD) combined with respiratory failure in patients with invasive and non-invasive ventilation. Methods: 120 cases of COPD complicated with respiratory failure were randomly divided into A group (60 cases) and B group (60 cases). B group were mechanically ventilated to weaning, and the A group were mechanically ventilated after invasive mechanical ventilation. The curative effect of the two groups were compared, and the incidence of ventilator associated pneumonia (VAP), the total mechanical ventilation time, the duration of ventilation, length of stay and the time of weaning were observed in the two groups. Results: The total effective rate, VAP incidence, total mechanical ventilation time, invasive ventilation time, hospital stay and weaning time were significantly better than that of A group (P<0.05). Conclusion: The sequential therapeutic effect of COPD combined with respiratory failure invasive and non-invasive ventilation is better than that of simple invasive respiratory unit.
