卵巢早衰的治疗新进展

New Progress in the Treatment of Premature Ovarian Failure

摘要: 卵巢早衰(POF)是指妇女在40岁之前即发生卵巢功能减退或者衰竭,临床上出现月经紊乱、潮热盗汗、阴道干燥等一系列雌激素低下症状,骨质疏松、神经系统疾病、心血管疾病等风险也增加,影响患者生活质量,此外有可能导致有生育要求的患者不能生育。截至目前,POF发病原因不明,可能与遗传因素、自身免疫因素、代谢因素、环境因素、医源性因素有关。目前激素替代治疗仍是POF的主要治疗方式,虽然可以改善患者临床表现,但无法恢复正常的卵巢功能,也无法达到患者的生育要求。为了改善这一状况,一些新型治疗方法正在发展并且取得了一定进展,如间充质干细胞(MSCs)治疗、富血小板血浆卵巢内输注、卵巢组织的体外激活、外泌体及miRNA治疗。本研究就目前卵巢早衰的治疗进展及其机制进行综述,以期为临床治疗POF提供借鉴和参考。

Abstract: Premature ovarian failure (POF) patients have ovarian dysfunction or even failure before the age of 40, and a series of low estrogen symptoms such as menstrual disorders, hot flashes and night sweats, vaginal dryness and so on appear clinically. The risk of osteoporosis, nervous system disease, cardiovascular disease and other diseases also increased, affecting the quality of life of the patients. In additon, it leads to patients with fertility requirements infertile. The etiology of most POF is unknown, which may be related to genetic factors, autoimmune factors, metabolic factors, environmental factors and iatrogenic factors. At present, hormone replacement therapy is still the main treatment method for POF, which can improve the clinical symptoms of patients, but can not restore the ovarian function and meet the fertility requirements of patients. In order to improve this situation, some new treatment methods are being developed and have made some progress, such as mesenchymal stem cell therapy, platelet-rich plasma intraovarian infusion, ovarian tissue activation in vitro, exosomes and miRNA therapy. This article reviews the current progress in the treatment of premature ovarian failure and its mechanism, so as to provide reference for clinical treatment of POF.
