临床药师参与治疗颅内感染继发碳青霉烯类耐药 肺炎克雷伯菌致重症肺炎的病例分析

摘要/Abstract

摘要： 目的：积累头孢他啶/阿维巴坦（CAZ/AVI）临床抗感染治疗经验，旨在为临床治疗多重耐药菌感染提供参考。方法：临床药师参与1例颅内感染继发碳青霉烯类耐药肺炎克雷伯菌（CRKP）致重症肺炎抗感染治疗的全过程，运用临床药学专业知识协助医师综合讨论分析，根据患者的实际情况合理选用药物，再依据药代动力学/药效学（PK/PD）原理精准给药。结果：使用CAZ/AVI 3 d后，患者体温及炎性指标有所下降，提示抗感染治疗有效。7 d后转回普通病房，19 d后治愈出院。结论：CAZ/AVI对CRKP感染治疗效果明确，临床药师可运用药学知识协助医师制定合理的治疗方案，实现个体化给药、精准治疗，实现临床药师的价值。

Abstract: Objective：To accumulate the clinical experience of ceftazidime/avibactam(CAZ/AVI) anti-infective therapy. Methods： Clinical pharmacists participated in the whole process of anti-infection treatment of a case of severe pneumonia caused by carbapenem-resistant Klebsiella pneumoniae (CRKP)， and used professional knowledge of clinical pharmacy to assist physicians to rationally select drugs according to the actual situation of the patient and then accurately administered according to PK/PD principle. Results：After 3 days of treatment with CAZ/AVI， the temperature and inflammatory indexes of the patient decreased， suggesting that the anti-infective therapy is effective. The patient was transferred to the general ward 7 days later， and was cured and discharged 19 days later. Conclusion： The therapeutic effect of CAZ/AVI on CRKP infection is definite. Clinical pharmacists can use pharmaceutical knowledge to assist physicians in making reasonable treatment plans， realize individualized drug thearpy and precise treatment， and realize the value of clinical pharmacists at the same time.
