氢吗啡酮用于带状疱疹后神经痛的研究进展

摘要：

带状疱疹后神经痛（postherpetic neuralgia，PHN）为带状疱疹皮疹愈合后仍持续4周以上的疼痛，是带状疱疹最常见的并发症，严重影响患者生活质量。PHN的治疗方法主要包括药物、微创介入、物理方法等。钙离子通道抑制剂、三环类抗抑郁药以及5％利多卡因贴剂为治疗PHN的首选用药，阿片类药物为其疗效不佳时使用的二线药物。氢吗啡酮为新型阿片类药物，镇痛作用强且不良反应少。近年来，诸多研究表明氢吗啡酮可有效缓解PHN患者的疼痛，改善生活质量，且具有较高安全性。本研究通过查阅相关文献，从氢吗啡酮的药理学、剂型、给药途径、在PHN中的应用以及药物不良反应等方面进行概述，总结了氢吗啡酮的特点与其在治疗PHN中的研究进展，为氢吗啡酮治疗PHN的临床应用和研究提供参考。

Abstract:

Postherpetic neuralgia （PHN） is characterized by pain that persists for more than four weeks after the resolution of herpes zoster skin rash. It is the most common complication of herpes zoster and significantly impairs the quality of life of patients. The treatment of PHN mainly includes medications， minimally invasive interventions， and physical methods. Calcium channel blockers， tricyclic antidepressants and 5% lidocaine patches are the first-line medications for treating PHN， while opioids are used as second-line drugs when the first-line treatments are ineffective. Hydromorphone， a novel opioid， has potent analgesic effect and with few side effects. In recent years， numerous studies have shown that hydromorphone can effectively alleviate the pain of PHN patients， improve their quality of life， and has a high safety profile. This study provides an overview of the pharmacology， formulations， routes of administration， application in PHN， and adverse drug reactions of hydromorphone， summarizs its characteristics and the research progress in the treatment of PHN， so as to provide references for the clinical application and research of hydromorphone in the treatment of PHN.
