酚妥拉明联合低分子肝素在肺心病急性加重期对血液高凝状态的影响分析

摘要: 【摘要】目的:观察酚妥拉明联合低分子肝素对肺源性心脏病肺心病急性加重期血液高凝状态的影响。方法:将104例肺心病急性加重期患者随机分入治疗组与对照组,两组患者均给予肺心病的常规治疗如氧疗、强心、改善通气、控制感染等;治疗组52例在常规治疗的基础上给予酚妥拉明与低分子肝素联合治疗,对照组52例在常规治疗的基础上给予低分子肝素钙治疗,比较治疗组与对照组血液高凝状态的改变。结果:治疗组(52例)与对照组(52例)患者治疗前全血高切粘度、全血低切粘度、血浆粘度、红细胞压积比、血小板聚集指数和纤维蛋白原比较均无显著性差异;治疗后,治疗组全血高切粘度、全血低切粘度、血浆粘度、红细胞压积比、血小板聚集指数和纤维蛋白原明显低于对照组,相比较具有统计学差异。结论:酚妥拉明联合低分子肝素治疗可显著改善肺心病急性加重期血液高凝状态,不良反应少,疗效满意。

Abstract: 【Abstract】Objective:To observe the effect of Phentolamine combined with low molecular weight heparin on hypercoagulability in patients with cor pulmonale in acute exacerbation stage. Methods: 104 patients with cor pulmonale in acute exacerbation stage were randomly divided into treatment group and control group. Two groups of patients were given conventional treatment of pulmonary heart disease such as oxygen therapy, strong heart, to improve ventilation, control of infection; 52 cases in treatment group treated with phentolamine and low molecular weight heparin on the basis of conventional therapy;trong heart, to improve ventilation, control of infection; 52 cases in treatment group treated with phentolamine and low molecular weight heparin on the basis of conventional therapy; 52 cases in control group were given conventional treatment on the basis of conventional therapy. Hypercoagulability was examined and compared between the two groups. Results:The treatment group?(52 cases)?and control group?(52 cases)?of patients before treatment?of whole blood high shear?viscosity,?whole blood shearviscosity,??plasmaviscosity,?hematocrit,?platelet?aggregation?and fibrinogen showed no?significant difference,?P > 0.05;?after treatment,?the treatment group?of whole blood high shear?viscosity,?whole blood low shear?viscosity, plasma viscosity,?hematocrit,?platelet?aggregation and fibrinogen?was obviously lower than that in control?group,?compared with statistical?difference, P < 0.01or0.05.Conclusion:Phentolamine combined with low molecular weight heparin in treatment of patients with cor pulmonale in acute exacerbation stage can significantly improve the hypercoagulability, which had less adverse reaction but had satisfactory effect.
