超声引导下针刀联合高渗葡萄糖注射治疗粘连性肩关节囊炎的临床研究

Clinical Study on the Treatment of Adhesive Capsulitis of the Shoulder Using Ultrasound-Guided Needle-Knife Combined with Hypertonic Glucose Injection

摘要:

目的:观察超声引导下针刀联合高渗葡萄糖注射治疗粘连性肩关节囊炎的临床疗效。方法:将80例粘连性肩关节囊炎患者随机分为对照组和试验组,各40例。两组患者均予以关节腔内高渗葡萄糖注射治疗,对照组患者予以传统针刀治疗,试验组患者予以超声引导下的针刀治疗,疗程均为每周1次,共治疗4次。治疗后采用肩关节功能评分(CMS)、视觉模拟评分法(VAS)、被动肩关节活动范围对患者肩关节疼痛程度、总体功能、活动范围进行评估,并判定两组患者的临床疗效及安全性。结果:治疗后试验组临床疗效高于对照组(P<0.05);治疗后两组患者VAS评分较治疗前均下降(P<0.05),且试验组下降更为明显(P<0.05);治疗后两组患者CMS评分均升高(P<0.05),且试验组升高更为明显(P<0.05);治疗后两组患者肩关节活动范围扩大(P<0.05),且试验组大于对照组(P<0.05)。两组患者均无明显不良事件发生。结论:超声引导下针刀联合高渗葡萄糖注射治疗能明显减少粘连性肩关节囊炎患者肩部疼痛,改善肩部功能,扩大肩部活动范围,安全有效。

Abstract: 【ABSTRACT】 Objective To observe the clinical efficacy of ultrasound-guided needle-knife therapy combined with hypertonic glucose injection in the treatment of adhesive capsulitis of the shoulder. Methods A total of 80 patients with adhesive capsulitis of the shoulder were randomly divided into a control group and an experimental group, with 40 patients in each group. Both groups were treated with hypertonic glucose injection in articular cavity, while the experimental group was treated with ultrasound-guided acupotomy, while the control group was treated with traditional acupotomy once a week, with a total of 4 treatments. Objective: To observe the clinical efficacy of ultrasound-guided needle-knife therapy combined with hypertonic glucose injection in the treatment of adhesive capsulitis of the shoulder.Methods: A total of 80 patients with adhesive capsulitis of the shoulder were randomly divided into the control group and the experimental group with 40 patients in each group. The clinical efficacy and safety of both groups were evaluated.Results: After treatment the clinical efficacy of the experimental group was higher than that of the control group P<0.05. Both groups showed reductions in VAS scores after treatment P<0.05), with a more pronounced decrease in the experimental group P<0.05. CMS scores increased in both groups after treatment P<0.05), with a greater increase in the experimental group P<0.05. The range of shoulder motion expanded in both groups after treatment P<0.05), with the experimental group showing a greater increase than the control group P<0.05. No significant adverse events occurred in either group.Conclusion: Ultrasound-guided needle-knife therapy combined with hypertonic glucose injection significantly reduces shoulder pain improves shoulder function and expands the range of shoulder motion in patients with adhesive capsulitis of the shoulder and the treatment is safe and effective.
