监护仪不良事件分析及比例失衡法在风险信号检测的应用

摘要:

目的:探索比例失衡法在监护仪不良事件风险信号检测中的应用价值。方法:利用山东省医疗机构2022年报告的监护仪不良事件数据,采用比例失衡法中的比例报告比法(PRR)、报告比值比法(ROR)、贝叶斯置信度递进神经网络法(BCPNN)和费舍尔精确检验(RFET4种方法检测监护仪不良事件风险信号。结果:监护仪不良事件主要是材料破损(包括导联线断裂和血压袖带破损漏气等)和采集捕获问题(主要是血氧探头问题),二者合计占比为51.2%。应用PRRRORRFETBCPNN 4种方法挖掘不良事件风险信号,按注册证编号分析,以上4种方法分别发现监护仪不良事件风险信号13个、8个、5个和5个;按注册人分析,以上4种方法分别发现不良事件风险信号6个、5个、5个和5个。结论:不良事件风险信号检测可发现医疗器械产品注册人与不良事件间可能存在的因果关系,便于早期预警。应用比例失衡法挖掘监护仪不良事件风险信号,可以发现风险企业(或注册证编号)和不良事件间的关联。

Abstract:

Objective: To explore the application value of the proportional imbalance method in risk signal detection of adverse events of medical monitoring instruments.Methods: Based on the data of adverse events reported by medical institutions in Shandong province in 2022 the proportional reporting ratio PRR), reporting odds ratio ROR), Bayesian confidence propagation neural network BCPNN), and reporting Fishers exact test RFET were used to detect the risk signals of medical monitoring instrument adverse events.Results: The adverse events of medical monitoring instruments were mainly material breakage including lead wire fracture and blood pressure cuff broken and deflated and collection/capture issues mainly blood oxygen probe problems), which accounted for 51.2% of the total. PRR ROR RFET and BCPNN were used to mine the risk signals of adverse events. According to registration certificate number the four methods identified 13 8 5 and 5 risk signals respectively and according to product registrant the four methods identified 6 5 5 and 5 risk signals respectively.Conclusion: Risk signal detection of adverse events can identify the possible causal relationship between the medical device product registrant and adverse events enabling early warnings. The proportional imbalance method can be applied to mine the risk signals of adverse events so as to identify the association between risk enterprises or registration certificate numbers and adverse events.
